Care Access Plan (CAP) is supplemental fixed indemnity insurance that includes a fixed benefit amount for covered medical expenses resulting from hospitalization, surgery, chemotherapy and radiation services.

There are three plan levels to choose from with a range of cost-sharing options for inpatient and outpatient surgeries, hospital confinement, inpatient physician visits, ambulance service, chemotherapy and radiation. Additional benefits can then be added on to further customize coverage.

With so many options for customization, there’s a CAP plan and benefits level to fit almost any budget and need.

Economy Fixed Benefits

Inpatient Services: $1,000 to $1,500

Inpatient Surgery: $3,000

Outpatient Services: $1,300

Value Fixed Benefits

Inpatient Services: $1,000 to $3,000

Inpatient Surgery: $6,000

Outpatient Services: $2,600

Primary Insured Lump Sum critical illness benefit: $10,000

Superior Fixed Benefits

Inpatient Services: $1,000 to $4,500

Inpatient Surgery: $9,000

Outpatient Services: $3,900

  • Plans pay the selected fixed benefits, regardless of the amount the provider charges.
  • Each plan level includes a lump sum critical illness benefit of $10,000 for the primary insured and spouse.
  • Benefits payments may be made directly to the provider or to the insured.

Available for ages 18 - 64.5

  • The primary applicant must be age 18 to 64.5 years old.
  • Dependents under age 18 may be included under a primary applicant.
  • Child-only plans are not available.

Plans are guaranteed-renewable

  • Insured and their family will not be singled out for a rate increase or cancellation based solely on health changes.

  • Not available without obtaining a major medical policy in some states.
  • Pre-existing condition limitation and 30-day waiting period for critical illness benefit rider.
  • There is an extended list of healthcare services and charges that are excluded from coverage.

Who May Want a Hospital Indemnity Policy?

  • Anyone with a high deductible major medical plan, including an individual ACA plan and employer group benefits.
  • People that are using supplemental insurance instead of ACA-qualifying major medical plan.
