Independence Hospital Insurance Plus is supplemental fixed indemnity insurance that includes a fixed benefit amount for covered medical expenses for a range of healthcare services, including inpatient and outpatient hospitalization, qualifying cancer diagnosis, as well as preventive care and physician’s office visits.

There are three plan levels to choose from with a range of benefits options:

Fixed benefits ranging from $1,000 to $4,000 depending on plan level selected

  • Plans pay the selected fixed benefits, regardless of the amount the provider charges
  • Includes a lump sum cancer benefit from $2,000 to $4,000
  • Benefits payments made directly to the insured, unless assigned to the provider

  • Inpatient observation unit
  • ICU/CCU confinement
  • Inpatient physician, surgeon, assistant surgeon and anesthesiologist
  • Outpatient surgeon, assistant surgeon and anesthesiologist
  • Outpatient diagnostic and advanced imaging
  • ER visit, ground, water and air ambulance
  • Second surgical opinion

Available for ages 18 - 64.5

  • The primary applicant and their spouse must be age 18 to 64.5 years old
  • Dependent children under the age of 26 may be added under a primary applicant

Plans are guaranteed-renewable

  • Insured and their family will not be for a rate increase or cancellation based solely on health changes
